What is the Prism Provider Directory?
For LGBTQ+ individuals in El Paso County, as well as those in search of gender-affirming care, finding mental and physical health care providers who offer sensitive, supportive, quality care has been an incredibly discouraging process for far too long. The Prism Provider Directory is dedicated to filling this crucial gap of care for our community.
Our free, searchable database is designed to connect the local LGBTQ+ community with inclusive healthcare professionals who prioritize LGBTQ+ and trans/nonbinary individuals and their unique needs.
How to join
Submit an Application
Fill out the application. There are both short and long form responses. It should take approximately 20-30 minutes to complete.
Upon receipt of the application, providers will be informed of their status in the review process.
Be Available for Follow-Up
We may have some additional questions, so be sure to look out for any follow ups we may have about your profile.
Once approved, you will be good to go!
Provide a Headshot
Once approved, send a headshot of yourself to directory@prismcos.org. The file size cannot exceed 500 KB.

Who created the directory?
The directory was developed by ChangeLine, a Colorado Springs-based nonprofit, through its LGBTQ+ Health Equity Initiative, which works to strengthen access to affirming care across the Pikes Peak region. It’s housed on the website of Prism Community Collective, a survivor- and LGBTQ+-led resource center serving the Colorado Springs community.
Who is eligible for inclusion in the directory?
The directory is open to mental and physical health care providers and health and wellness practitioners who are already working with LGBTQ+ patients and clients, particularly those with experience or a focused interest in trans and queer health. If you’re newer to this work but committed to growing your skills, send an email to directory@prismcos.org and a member of ChangeLine’s LGBTQ+ Health Equity team will connect with you to talk through next steps.
Why join the directory?
Being listed means your practice is visible to an LGBTQ+ community that’s actively looking for affirming care. The directory has received tens of thousands of page views since launching in 2024.
But visibility is just part of it. Listed providers also gain access to free or low-cost training opportunities and a growing network of affirming providers to learn from, share resources with, and build referral relationships alongside. For many providers, that matters as much as anything else. They’re deeply committed to serving LGBTQ+ patients but have felt uncertain or isolated in that work. Being part of this network means they don’t have to figure it out alone.
Is there a cost?
No. Joining the directory is completely free.
How do I become part of the directory?
Start by filling out an application!
The application takes about 20-30 minutes to complete. If you need to pause and return to it, email directory@prismcos.org and an edit link can be sent to you to resume where you left off.
A couple of technical notes: The form works best in Microsoft Edge, and if you run into trouble uploading your headshot, you can send it directly to Melissa instead.
What happens after I apply?
Once you submit your application, the LGBTQ+ Health Equity team will review it to assess readiness for inclusion. You can expect to hear back within 10 business days with either an approval or information about next steps. When you’re added to the directory, we’ll let you know.
I'm not sure I'm ready to join the directory but would still like to be involved. What are my options?
We offer trainings and networking opportunities for providers who want to build their skills and connections before joining the directory. Send an email to directory@prismcos.org to learn more.
