Funding Sources
Prism Community Collective is primarily being funded through state, federal and foundation grants that fund institutions to build capacity, develop resources, and address disparities in health equity. Any individual donations received help pay for services and resources.
Anti-Terrorism Emergency Assistance Program
Community Health Partnership and the Office for Victims Programs worked together to request a grant from the Anti-Terrorism Emergency Assistance Program to support Prism Community Collective for three years.

Partnerships
Community Health Partnership
Community Health Partnership (CHP) is the lead agency and fiscal sponsor for Prism Community Collective. A local nonprofit, its mission is to transform the way people work together to solve complex community health issues. CHP's role in this effort is to support individuals and organizations as they co-create transformative solutions. This is accomplished by building essential skills and capacities in the community, incubating ideas, fostering proactive innovation, collaborating with diverse voices, and facilitating win-win outcomes. CHP’s focus work includes LGBTQ+ health, homelessness, behavioral health, and suicide prevention.
NAMI
NAMI provides advocacy, education, support and public awareness so that all individuals and families affected by mental illness can build better lives.
Peak Vista
Peak Vista is a nonprofit Federally Qualified Health Center that provides medical, dental, and behavioral health care services.
Diversus
Diversus Health Services provides counseling, behavioral health, mental health, and crisis support services in Colorado.
Hey719 / Southern Colorado Health Network
The mission of Hey719 is to improve the sexual health and overall well-being of all members of the Colorado Springs community, particularly those experiencing systems of oppression that impede access to testing, PrEP, and other resources. Its parent organization, Southern Colorado Health Network, is part of a statewide organization that serves individuals living with HIV by providing a broad spectrum of holistic support services.
