Need help navigating primary care providers, mental health, and dentistry? Diversus Health sends an Outreach Specialist to Prism the 2nd and 4th Thursday of every month from 2pm – 4pm! Outreach Specialists can help you navigate Diversus Health’s vast options for all things health! Come down to Prism and meet with the Diversus Health Outreach Specialist today!
The Outreach Specialist can also help you get registered for the monthly group therapy held on the last Thursday of every month!
New to Prism? Here’s what to expect:
- Our doors are always locked for security. We have an intercom at the door as first point of contact. Simply call “front office” from the touch screen.
- Weapons are not allowed in space.
- There is a preliminary security check at the door
- We have an intake form we ask all new folks to fill out. Try to arrive a few minutes early to fill that out.
